VERMONT COMFORTS OF HOME – SOUTH
Home is where the Heart is . . . Vermont Comforts of Home (VCOH-S) provides opportunities for people to maintain their independence, safety, security and relationships in a personalized setting.
At times, families struggle to care for a loved one who needs more support, while also being challenged with having to work outside the home to ensure needed income. If your loved one is eligible for the State of Vermont’s Adult Family Care program, a member of their family can be compensated to care for them in their home, with the outcome of keeping family together, ensuring income and avoiding a nursing home or hospital placement.

At some point in our lives, we all need a helping hand. VCOH believes that being in a home environment, in familiar surroundings and with people who care about us, promotes wellness. If a family is unable to care for one of its members, we at VCOH-S can provide the support and services you need in a setting that offers all the Comforts of Home. Our contracted home providers are skilled in providing care, have complete background checks and are trained and supported by VCOH-S Care Coordinators who closely monitor the quality of care a family member receives.
Vermont Comforts of Home South is operated by Lincoln Street Inc., and operates in Windsor, Windham, Rutland, Addison, Orange and Bennington counties. Vermont Comforts of Home North is operated by Upper Valley Services in central and northern Vermont counties. Both organizations are regulated, funded and monitored for quality of services by the State of Vermont Department of Aging and Independent Living.

Your life. Your choices. Our supports!

Who is Eligible for Vermont Comforts of Home Services?
If someone you know is 18 or older and needing help with such issues as medical care, mobility, personal care, medications, decision-making and safely accessing their community, then they might be eligible for this program. If and when, for a variety of reasons, someone is incapable of managing on their own in a home environment, moving into a family member’s home, or with one of VCOH Home Providers, can be the perfect solution for more personalized care.
VCOH-S operates under the eligibility, funding and quality measures of the Adult Family Care Program, which, itself, is available through Vermont’s Choices for Care Program and is funded through Medicaid for those people clinically and financially eligible. There is also a private pay option available for VCOH-S participants. If you are interested in choosing a personalized alternative to nursing home care, please call us for details.
