If you or someone you know is seeking information regarding eligibility for an individual with intellectual and developmental disabilities, Lincoln Street Inc. may be just the resource you need. As a Specialized Service Agency, LSI has developed expertise in a variety of areas, in addition to general disability services.

Over time, we have developed expertise to support people who do not use spoken language, as well as those with complex medical conditions, including autism, dementia, eldercare and end-of-life care. We also are recognized for our quality services for youth in transition from school to adult services, including our partnership with Project SEARCH at Dartmouth Hitchcock Medical Center, Twin State Youth Transition services for New Hampshire students attending SEARCH, Global Campus, and our supported apartment program.

We can help guide you through the eligibility process at the designated agencies, work with you to identify your needs, and help secure the funding necessary for your needed services.

Baseline criteria for eligibility at LSI include meeting both financial and clinical eligibility requirements. These criteria may be found on the Vermont Department of Aging and Independent Living website, and at a minimum include:

BASELINE CRITERIA FOR VERMONT SERVICES ELIGIBILITY AT LSI INCLUDE THE FOLLOWING:

  • An individual was diagnosed with an intellectual/developmental disability prior to the age of 18 (i.e., significantly sub-average cognitive functioning documented by a full scale IQ score of 70 or below), accompanied by deficits in two or more areas of adaptive behavior skills, or . . .
  • An individual must have been diagnosed with an autism spectrum disorder prior to age 18, accompanied by deficits in two or more areas of adaptive skills.

In addition, an individual’s needs must meet a current funding priority within the State System of Care Plan, as identified by Vermont Department of Disabilities, Aging and Independent Living (DAIL).

Evaluations to determine clinical eligibility are completed via a Designated Agency. If you represent a Specialized Service Agency and are choosing LSI, we will assist you through this process as well as securing funding to meet your service needs.
