ACCESSING OUR SERVICES

To be found eligible, individuals must be a Vermont resident, a current Medicaid recipient, and meet clinical eligibility based on the Regulations Implementing the Developmental Disabilities Act (HCAR 7.100), including a diagnosis of an intellectual and/or developmental disability (I/DD). Additionally, each program has unique eligibility requirements. More information about these requirements can be found on the DAIL website.

During the intake meeting with you, the PCG Intake and Eligibility team will collect information to determine whether an individual meets financial, clinical, and program access eligibility requirements.
The PCG Intake and Eligibility team can answer questions about DDS programs and eligibility criteria, help individuals and their families complete an application, arrange for clinical eligibility determinations and arrange for needs assessments, as necessary.

This team will also provide newly enrolled individuals with information needed to select a Case Management Organization, either Benchmark or Columbus. The Case Management Organizations will work with you to choose a service provider such as Lincoln Street, work with you and LSI on your ISA, budget and service needs.
